WE are a trusted community resource for free mental and physical health services that heal and prevent homelessness, unemployment, broken relationships, and suicide.

Before Starting - Applications must be completed in one session and cannot be saved/resumed later. Prior to beginning, ensure you have the following ready:

  1. A current copy of your insurance document. Please note you may need to scroll to the top of the popup window to upload your document.
  2. Your license number
  3. (optional) a digital, professional photo for your directory listing
  4. Interns only - Your supervisor’s information (name, email, and license number). We will contact your supervisor to obtain their approval for joining the network.
